Request Claims Histories & Loss Runs Here
If you are working on an application or credentialing with a hospital or medical center, you may need a claims history or loss run.
If you are in need of either of these reports, you can either submit the request form below, or contact our office directly to speak with the service specialist on your account. Please note, if you're a Professional Risk Management Services insured, you will need to reach out to them directly as we are not affiliated.
If you're from a Credentialing Department or trying to submit a request for someone other than yourself, please email your request and signed authorization of release to claimshistoryrequests@profrisk.com.
